Introduction

Here are screenshots from the Record a Note system I developed for a private party. Click on the image to view a larger screenshot.

Main Screen

Here is the main screen. From here nurses can login to add new notes, visitors can view product details and administrators can view note reports.

Note Entry Page

Here a sample note page. Nurses login and are brought to this page for data entry of a patients vitals and other treatment notes. All data here is stored in a MySQL database for processing during report generation time.

Report PDF

Here Nurses can further refine patient notes by making changes. Administration staff can view patient records from previously entered notes. Note that printing is allowed and design of these forms closely approximates their current form.
